A well-organized 468 pages and very good. This is WORLDWIDE herbology, the best herb for the job, irrespective of country or culture. Does not address the energetic angle of interpreting illness as Messages from the Body, but short of that, the best textbooks I've ever seen on herbs for antibiotic use.

The below is a view of this work from an energetic persepctive; that is, what can be tested with muscle testing of any kind, towards solutions for your self and for patrons.

General rules of thumb for dealing with resistant infections

Systemic infection ~ try: Cryptolepis
Severe diarrhea, dysentery ~ try berberine herbs, any
Urinary tract infection ~ try: Juniper berry combined with bidens
Infected surface or surgical wound ~ try: Honey has always worked
Menningitis ~ try: Add piperine, isatis and others are suggested

(abbreviated from p. 45)

Buhner reminds of the distinction between bacteria with one cell wall and two cell walls. Gram positive bacteria are stainable because they have only one cell wall. Gram negative bacteria are not stainable as they have two cells walls. This distinction is therefore a possible distinction to test for.

The main resistant Gram-positive (single cell wall) bacteria are:
- Clostridium difficile
- Enterococcus spp.
- Mycobacterium tuberculosis
- Staphylococcus aureus
- Streptococcus spp.
(abbreviated from Chap. 2)
Extensive detail on how to deal herbally with each and its variats follows.

The main resistant Gram-negative (double cell wall) bacteria are:
acinetobacter baumannii
campylobacter jejuni
E. coli
haemophilus influenzae
klebsiella pneumoniae
neisseria gonorrhoeae
proteus spp.
I have the earlier version and was delighted to get this second edition recently, three times the size of the original. The author states: "In the years since I wrote the first edition of this book, my knowledge of plant medicines and their use in healing has increased tremendously. Thus this new edition...is a great deal more comprehensive..." I found the first edition to be excellent and this one is even better. I also loved Buhner's "The Secret Teachings of Plants".

Here Buhner offers a slightly different list of the "top antibiotic herbs" than the first edition as deeper experience has shown him that, for instance, garlic and grapefruit seed extract are just not as effective as some of the new herbs in this edition. (I was pleased to read this as it reflects my own disappointing experiences with these two herbs.) Here the herbs have been divided into "the systemics" (cryptolepis, sida, alchornea, bidens, artemisia); "the localised nonsystemics" (berberines, juniper, honey, usnea); "the synergists (licorice, ginger, black pepper/piperine); and 8 herbs that he describes as the first line of defense (strengthening the immune system). There are a few tips on virus, fungus and parasite infections, although it does not really attempt to cover these pathogens. At the end are invaluable and detailed how-to-make instructions as well as a "formulary" (tincture proportions and dosages).

Thankfully, this book is not one of those encyclopedia-type listings that leave you wondering what herb/s to shortlist and where and how to actually start applying the information. With such books it is all little more than a lottery - perhaps slightly more educated than a lucky dip requires, but still guesswork.

This book is the second, revised edition of a practical handbook of Chinese medical patterns with their disease causes and mechanisms, signs and symptoms, treatment principles, guiding formulas, main modifications, and acupuncture treatments. It is meant for both the student and the clinical practitioner. The authors have included numerous patterns previously not described in the English language literature as well as many complex patterns which are commonly seen in real-life practice in the West. It also includes a symptom-sign index and a formula index for easy reference. All medicinal names in this new edition conform to Bensky et al.'s Chinese Herbal Medicine: Materia Medica 3rd edition, and all Chinese medical terminology corresponds to Wiseman and Feng's A Practical Dictionary of Chinese Medicine. Ingredients in Chinese medicinal formulas begin with their pinyin names and are listed down for easier reference. In addition, this new edition contains a number of case histories for use in problem-based learning (PBL) along with suggested answers, thus making it even more useful in the classroom.

The book description is misleading:

"It includes many of the complicated patterns other English language TCM books leave out but which Western patients typically present. You'll also learn Bob Flaw's personal insights on the discrimination of patterns and their treatment based on his more than 20 years of clinical experience and research."

Based on this description, I was expecting to see many patterns not discussed in the Foundations textbook as well as commentary from a Western viewpoint.

Instead, I found that most of the patterns in the book were the standard zang fu patterns listed in a similar format as the Foundations book, only described more superfluously. In my opinion, there was not enough added value to justify purchasing this book.

I went through the book looking for "Bob Flaw's personal insights" and this amounted to a few pages at the back of the book and a sentence every 5 patterns saying something like, "this pattern is not commonly seen" for something that you already know is not commonly seen.
By Lisa W.
Concise portable reference guide to a wide number of patterns, many of which are not featured in CAM or Giovanni. Additionally, it is written in Nigel Wiseman's translation of Chinese medical terms, which are much more faithful to the Chinese. With characters and pinyin for each pattern, it makes easy reference for those also studying the Chinese language (something else missing from Giovanni's books).

In addition to the pattern reference guide, it includes useful commentary from the author on certain patterns, especially on those that are difficult to treat or flesh out. Also it has nearly 30 pages of case studies that help students understand the patterns better, as well as how to translate Bio-medical diseases into Chinese Medicine patterns.

I almost didn't buy this book due to a bad review, but upon buying it I see that its more than worth the money. A useful addition to any Chinese medicine library

If you need the interactive nutrition program to figure out a diet plan, get the text as it is included, otherwise you will need to buy that access code separately and it will end up costing you the same as the hardcover.
By mya noelle
I seldom come upon textbooks that are simultaneously interesting to read and have loads of applicable information--usually, these factors are inversely related. But the writing in this textbook is smart, easy to read, and subtly humorous, which really makes a difference for a college student who is drowning in a million essays and texts for other classes. Every time I open these pages, I learn new, fascinating things about the human body and its intimate relationship with food that I've never cared to think about before. The textbook also clarifies a lot of misguided health and nutrition issues that the food industry and media always love to distort. You can definitely tell that the authors don't just care about the physical aspects of living healthfully--they emphasize the importance of a healthy mental process as well. Their entire approach is holistic and thoughtful. I love that I can close the book after reading about practical steps to effectively create a healthy lifestyle, get up from my desk, and immediately put my new knowledge to use. For anyone who's ever dieted or wanted to get fit, you can forget the fad diets, juicing/detox plans, and weight loss books. All you really need and probably all you'll ever need is this textbook, which provides accurate, research-based information. This book is the real deal, and you'll most likely never feel the need to peruse the weight loss shelves at Barnes and Noble or feel completely clueless about nutrition and health ever again.

THE MOST PRACTICAL, UP-TO-DATE GUIDE TO MODELLING AND ANALYZING TIME-TO-EVENT DATA—NOW IN A VALUABLE NEW EDITION

Since publication of the first edition nearly a decade ago, analyses using time-to-event methods have increase considerably in all areas of scientific inquiry mainly as a result of model-building methods available in modern statistical software packages. However, there has been minimal coverage in the available literature to9 guide researchers, practitioners, and students who wish to apply these methods to health-related areas of study. Applied Survival Analysis, Second Edition provides a comprehensive and up-to-date introduction to regression modeling for time-to-event data in medical, epidemiological, biostatistical, and other health-related research.

This book places a unique emphasis on the practical and contemporary applications of regression modeling rather than the mathematical theory. It offers a clear and accessible presentation of modern modeling techniques supplemented with real-world examples and case studies. Key topics covered include: variable selection, identification of the scale of continuous covariates, the role of interactions in the model, assessment of fit and model assumptions, regression diagnostics, recurrent event models, frailty models, additive models, competing risk models, and missing data.

Features of the Second Edition include:

  • Expanded coverage of interactions and the covariate-adjusted survival functions
  • The use of the Worchester Heart Attack Study as the main modeling data set for illustrating discussed concepts and techniques
  • New discussion of variable selection with multivariable fractional polynomials
  • Further exploration of time-varying covariates, complex with examples
  • Additional treatment of the exponential, Weibull, and log-logistic parametric regression models
  • Increased emphasis on interpreting and using results as well as utilizing multiple imputation methods to analyze data with missing values
  • New examples and exercises at the end of each chapter

Analyses throughout the text are performed using Stata® Version 9, and an accompanying FTP site contains the data sets used in the book. Applied Survival Analysis, Second Edition is an ideal book for graduate-level courses in biostatistics, statistics, and epidemiologic methods. It also serves as a valuable reference for practitioners and researchers in any health-related field or for professionals in insurance and government.

Index.

The authors provide a really nice, non-technical survey of the landscape for Cox Proportional Hazards models. A nice aspect of their treatment is the care they take to reference all highly technical texts and journal articles. For example, if you'd like to find out more about goodness-of-fit tests for survival models, the authors provide ample references to the Counting Process Theory of Martingale Residuals.

The first chapter discusses the basic characteristics of survival data, including the notion of censoring (in all of its various forms). Examples of the principle types of censoring are included. The chapter also includes introductory material on the general survival model, including a nice description of the log likelihood function. Curiously, the rigorous definition of the hazard function has been omitted, probably to avoid intimidating readers who are not familiar with formal limits.

Chapter 2 continues to build up the general survival model and introduces the relationship between the survivor function and the cumulative hazard. Pointwise estimators for the survivor function are discussed, including the Kaplan-Meier estimator along with the various variance estimators. Test statistics for comparing two survival populations are introduced, including the Log-Rank and General Wilcoxon statistics. The reader is encouraged to read the counting process treatments of these statistics to see why they produced defensible hypothesis tests.

Chapter 3 is devoted to the Cox Model and Cox's partial likelihood function. Tests for significance of the coefficients are introduced, included the Wald test, log likelihood ratio test and the score test. These are used heavily in the later chapters as the basis of a model-building methodology.

This book makes Medicinal Chemistry make sense. It's laid out in easy to digest chapters with lots of examples and practice problems. Other books I've read on the subject aren't written for someone with not a lot of background in Chemistry, so they were difficult and muddy. Harrold and Zavod did a great job providing a breakdown of the fundamental pieces of Medicinal Chemistry. I honestly enjoyed reading this book and understand so much more of the foundational pieces I need for my continuing years in Pharmacy School. I would highly recommend everyone that's looking for a place to start or to solidify their skills to read this book. It's the one book I will definitely use over again.

Science journalist Jessica Wapner vividly relates the backstory behind those headlines, reconstructing the crucial breakthroughs, explaining the science behind them, and giving due to the dozens of researchers, doctors, and patients whose curiosity and determination restored the promise of a future to the more than 50,000 people diagnosed each year with chronic myeloid leukemia (CML). It is an astonishing tale that will provide victims of other cancers and their loved ones realistic hope that cures may yet be found in their lifetimes.

The Philadelphia Chromosome charts the milestones that led to present-day cancer treatment and tells the inspiring story of the dedicated men and women who, working individually and in concert, have sought to plum the mysteries of the human genome in order to conquer those deadly and most feared diseases called cancer.

Reading the Philadelphia Chromosome transformed me into a mini scientist majoring in CML, Chronic Myelogenous Leukemia. I was diagnosed with CML in November of 2003, which required keeping up-to-date on news relating to CML. When I heard about the Philadelphia Chromosome by Jessica Wapner, I was anxious to add it to my shelf of resources.

Reading the book with pencil in hand to highlight new facts as well as valuable previous knowledge, I found myself marking information on every page.

When I was diagnosed my oncologist informed me that if there was ever a good time to get CML, it was now. At that precise moment, I had no idea what he was talking about. He may have elaborated, but in that moment of shock, I didn't hear much. Wapner's book has renewed my appreciation of that conversation every time I swallow my oral chemotherapy pill, Gleevec.

I have an entire file cabinet filled with lab results since 2003. My oncologist reviews the findings with me twice a year, but after reading the Philadelphia Chromosome, my understanding of the labs has improved. I have registered for a couple of CML conferences and am confident I will easily grasp new information presented after reading this book.

Years ago I started writing a book about living with CML. I found it too depressing to continue, however, not abandoning the therapeutic effect; I turned it into a blog, which I update once a month. marycrocco.wordpress.com Being helpful to a few readers who have stopped by makes it worthwhile.

Wapner shared a story of a patient who cherished her Gleevec and defended it with her life. I do the same thing, always insisting to sign for it and checking the delivery time is set for the morning.
This book is divided into roughly three parts. The first part consists of an account of how scientists tracked a certain kind of leukemia down to its genetic origins. Some of the explanation here can be a little difficult. But don't be put off. While this section doesn't necessarily make for light bedtime reading - it is accessible. Wapner presents the material in short, easily digestible chapters. She frequently reviews what's been covered so far, and she is a master of writing clear, logical sentences that include analogies from everyday life. So you will readily enough be able to understand this section if you just turn off the TV and concentrate for short stretches.

Your diligence will be rewarded. At the end of this section, you'll have a clear understanding of how this type of cancer, and of how cancers in general, can arise in the very core of our cells. You'll also understand how a drug can be tailored to specifically stymie the action of the aberrant chromosomes.

The second section deals with how Brian Druker and others ushered such a drug through the necessary test phases. Reading about this often frustrating process, you'll learn how tests must progress from animal to human subjects in order to eventually try for FDA approval. The pill that was eventually formulated proved to be so much more successful at fighting back the cancer than previous drugs or procedures, it was fast-tracked for FDA approval. Here the reading gets easier, although perhaps not quite as interesting.

Finally, Wapner recounts how the drug was named Gleevec and was marketed. She also tells how this pioneering drug has opened the door to what's become a flood of other drugs targeted at blocking the chemical chain of events that arise from a mutated gene.

This is such a fantastic book as an introduction to the concepts of achieving good exposure in orthopedic surgery!

Of particular note to medical students about to start buying books for this fall's away rotations: I didn't, and I wouldn't, really fool much with Netter's Concise Orthopaedic Anatomy, 2e (Netter Basic Science), since the level of detail just doesn't compare. If you're gonna read about an exposure, you may as well do it right and read about it in Hoppenfeld. Throw down the coin, since--after all--you'd expect that you'll be needing this book for a few years to come anyway! Look at it as an investment in the Match! Read on for reasons why...

I write this as an about-to-start intern who used this book extensively on my third- and fourth-year orthopedics rotations during medical school, so it is mostly medical students who will find this review useful. This book almost definitely was over my head in large parts during my third year but started to click early in my fourth-year rotations after I had spent a good bit of time in the OR. I see residents referring to this book often, even the seniors sometimes, for a great broad overview of the "large menu of options" to "get to the bone and stay there", especially for fractures. Discussion of the rationale for particular approaches is somewhat limited (and is probably a discussion more suited to books written about particular injuries that would spark such a debate), but the "how-to" and the lending of understanding of the anatomy while in a particular approach is spectacular.

This manual presents the first empirically studied, integrative treatment approach developed specifically for PTSD and substance abuse. For persons with this prevalent and difficult-to-treat dual diagnosis, the most urgent clinical need is to establish safety--to work toward discontinuing substance use, letting go of dangerous relationships, and gaining control over such extreme symptoms as dissociation and self-harm. The manual is divided into 25 specific units or topics, addressing a range of different cognitive, behavioral, and interpersonal domains. Each topic provides highly practical tools and techniques to engage patients in treatment; teach "safe coping skills" that apply to both disorders; and restore ideals that have been lost, including respect, care, protection, and healing. Structured yet flexible, topics can be conducted in any order and in a range of different formats and settings. The volume is designed for maximum ease of use with a large format, lay-flat binding, and helpful reproducible therapist sheets and handouts.

This book, intended for therapists, is a comprehensive, compassionate, and accessible guide to what Najavits establishes is the first crucial step for someone struggling with PTSD and substance abuse: learning to feel safe. It is also an excellent resource for patients who want to help themselves. What I like best is that the book is filled with checklists, stories, exercises, and quotations such that I can open to nearly any page and find ideas that are both practical and inspirational. In short, this is a great book.

I can see why this book got such rave reviews and is such a best-seller. It's a superb book, offering both real insight into trauma and addiction, as well as hands-on clinical tools. As someone in the field, I have found both the philosophical and practical aspects of Najavits' work incomparable. On top of the impeccable professionalism, is her impressive writing. This is a must buy for anyone interested in this area.

Designed for advanced practice nurses and advanced practice nursing students, as well as Physician's Assistant students and practitioners, Advanced Health Assessment & Clinical Diagnosis in Primary Care, 4th Edition, is a practical resource that takes you to the "next step" of health assessment, beyond basic history and physical examination and through the diagnostic reasoning process. Accessible and concise, it approaches physical examination by focusing on a specific chief complaint rather than a diagnosis of a disease entity. Each chapter is organized into four major areas: Focused History; Focused Physical Examination; Laboratory and Diagnostic Studies; and Differential Diagnosis. Those who master the diagnostic reasoning process in this text will be able to accurately diagnose the majority of conditions they will see in clinical practice.

  • Easy-to-follow format with consistent organization improves your ability to understand and accurately perform the different elements of the diagnostic reasoning process:
    • Focused History sections walk you through the thinking process involved in obtaining a pertinent, relevant, problem-specific history that will assist in differential diagnosis.
    • Key Questions highlight what questions to ask the patient, followed by an explanation of what the patient's responses might signify, to guide you toward an accurate assessment and precise diagnosis.
    • Focused Physical Examination sections explain how to conduct more advanced diagnostic techniques and offer interpretations of the findings.
    • Laboratory and Diagnostic Studies sections give a brief outline of what types of laboratory or diagnostic studies would be appropriate for the chief complaint or suspected diagnosis.
    • Differential Diagnosis sections contain the most common differential diagnoses for each chief complaint and summarize the history and physical examination findings, along with the laboratory and diagnostic studies indicated.
  • Differential Diagnosis tables offer an at-a-glance summary of possible diagnoses.
  • Reordered table of contents, organized alphabetically by patient problem rather than by body system, simplifies and accelerates information retrieval. A list of chapters by body system is also included for reference.
  • Three new chapters:
    • Chapter 23: Palpitations
    • Chapter 36: Weight Loss/Gain (Unintentional)
    • Chapter 38: The Abdominal X-ray
  • Additional Evidence-Based Practice boxes provide additional research-based tips on conducting the most effective exams for more accurate diagnoses.

Great review text, but wouldn't recommend for sole source of information. However the practice problems are top notch. For more in-depth medicinal/pharmaceutical chemistry try Silverman instead.

Philadelphia, 1959: A scientist scrutinizing a single human cell under a microscope detects a missing piece of DNA. That scientist, David Hungerford, had no way of knowing that he had stumbled upon the starting point of modern cancer research— the Philadelphia chromosome. It would take doctors and researchers around the world more than three decades to unravel the implications of this landmark discovery. In 1990, the Philadelphia chromosome was recognized as the sole cause of a deadly blood cancer, chronic myeloid leukemia, or CML. Cancer research would never be the same.

Science journalist Jessica Wapner reconstructs more than forty years of crucial breakthroughs, clearly explains the science behind them, and pays tribute—with extensive original reporting, including more than thirty-five interviews—to the dozens of researchers, doctors, and patients with a direct role in this inspirational story. Their curiosity and determination would ultimately lead to a lifesaving treatment unlike anything before it.

The Philadelphia Chromosome chronicles the remarkable change of fortune for the more than 70,000 people worldwide who are diagnosed with CML each year. It is a celebration of a rare triumph in the battle against cancer and a blueprint for future research, as doctors and scientists race to uncover and treat the genetic roots of a wide range of cancers.

A standard textbook for 25 years, Surgical Exposures in Orthopaedics: The Anatomic Approach is now in its Fourth Edition. Featuring 775 full-color illustrations, this atlas demonstrates the surgical approaches used in orthopaedics and provides a surgeon's-eye view of the relevant anatomy. Each chapter details the techniques and pitfalls of a surgical approach, gives a clear preview of anatomic landmarks and incisions, and highlights potential dangers of superficial and deep dissection.

The Fourth Edition describes new minimally invasive approaches to the spine, proximal humerus, humeral shaft, distal femur, proximal tibia, and distal tibia. Other highlights include new external fixation approaches for many regions and surgical approaches to the os calcis. New illustrations of the appendicular skeleton are included. New drawings show the important neurovascular structures that need to be protected.

In the 1890s, a proofreader at the University of Chicago Press prepared a single sheet of typographic fundamentals intended as a guide for the University community. That sheet grew into a pamphlet, and the pamphlet grew into a book--the first edition of the Manual of Style, published in 1906. Now in its fifteenth edition, The Chicago Manual of Style--the essential reference for authors, editors, proofreaders, indexers, copywriters, designers, and publishers in any field--is more comprehensive and easier to use than ever before.

Those who work with words know how dramatically publishing has changed in the past decade, with technology now informing and influencing every stage of the writing and publishing process. In creating the fifteenth edition of the Manual, Chicago's renowned editorial staff drew on direct experience of these changes, as well as on the recommendations of the Manual's first advisory board, composed of a distinguished group of scholars, authors, and professionals from a wide range of publishing and business environments.

Every aspect of coverage has been examined and brought up to date--from publishing formats to editorial style and method, from documentation of electronic sources to book design and production, and everything in between. In addition to books, the Manual now also treats journals and electronic publications. All chapters are written for the electronic age, with advice on how to prepare and edit manuscripts online, handle copyright and permissions issues raised by technology, use new methods of preparing mathematical copy, and cite electronic and online sources.

A new chapter covers American English grammar and usage, outlining the grammatical structure of English, showing how to put words and phrases together to achieve clarity, and identifying common errors. The two chapters on documentation have been reorganized and updated: the first now describes the two main systems preferred by Chicago, and the second discusses specific elements and subject matter, with examples of both systems. Coverage of design and manufacturing has been streamlined to reflect what writers and editors need to know about current procedures. And, to make it easier to search for information, each numbered paragraph throughout the Manual is now introduced by a descriptive heading.

Clear, concise, and replete with commonsense advice, The Chicago Manual of Style, fifteenth edition, offers the wisdom of a hundred years of editorial practice while including a wealth of new topics and updated perspectives. For anyone who works with words, whether on a page or computer screen, this continues to be the one reference book you simply must have.

What's new in the Fifteenth Edition:

* Updated material throughout to reflect current style, technology, and professional practice

* Scope expanded to include journals and electronic publications

* Comprehensive new chapter on American English grammar and usage by Bryan A. Garner (author of A Dictionary of Modern American Usage)

* Updated and rewritten chapter on preparing mathematical copy

* Reorganized and updated chapters on documentation, including guidance on citing electronic sources

* Streamlined coverage of current design and production processes, with a glossary of key terms

* Descriptive headings on all numbered paragraphs for ease of reference

* New diagrams of the editing and production processes for both books and journals, keyed to chapter discussions

* New, expanded Web site with special tools and features for Manual users. Sign up at www.chicagomanualofstyle.org for information and special discounts on future electronic Manual of Style products.

Because of the size of the print used in the 15th edition, I could not read the text so I rushed downtown (14 miles one way) and bought a stronger pair of reading spectacles. With the new glasses, I noted strange looking numbers announcing each paragraph. Paragraphs 1.33 and 3.11 looked as though they were set by an inebriated typesetter. The "one" numerals in both paragraphs appeared to be superscripted.
My curiosity led me to the colophon where I learned the manual is set with Scala and Scala Sans fonts. I used my favorite search engine and discovered these fonts were "invented" in 1999. I also discovered numerals 1, 2, and 0 are diminutive and the remainder are mega size and numerals 6 and 8 are superscripted. Mystery solved: The Chicago Press did not have an inebriated type setter.
Even with my more powerful spectacles, I still had trouble reading the text. I used my microscope and measured the capital letters in the 14th edition; they are 0.2 millimeters taller than the Scala capitals. This seems trivial, but it is not (particularly for older writers and editors). More bothersome than the small type size is the bluish type found in chapter 5. At first I thought there were sections of the text missing. All the text within the curly brackets is printed with barely discernable blue ink, which is virtually invisible under certain artificial light conditions. On page 148, note 5, we are told the curly brackets were used to save space. I don't have the Scala on my Mac but I tested several commonly used fonts and discovered words and phrases enclosed in square brackets use less space!
On the plus side Chicago has seen the light and dumped a couple of their old, long-standing edicts that made little or no sense. They now recommend the month-day-year method of recording dates (p.
